CRAFFT 2.1 Screening Tool
The CRAFFT 2.1 is a well-validated substance use screening tool for adolescents aged 12-21. It is recommended by the American Academy of Pediatrics’ Bright Futures Guidelines for preventive care screenings and well-visits. The CRAFFT 2.1 includes six questions: Car, Relax, Alone, Forget, Friends, Trouble. The patient is asked to answer "Yes" or "No" to each question. It is a short, effective screening tool meant to assess whether a longer conversation about the context of use, frequency, and other risks and consequences of alcohol and other drug use is warranted.
