  4. Simple fall
    Fall from > 3 feet or > 5 stairs
    Motor vehicle collision
    Other (e.g. abuse, sports injury)
  5. Is there vomiting?
  6. No
    Yes, once
    Yes, > 1 time
  7. Is there severe headache?
  8. No
    Yes
  9. Is there loss of consciousness?
  10. No
    Yes, < 5 seconds
    Yes, ≥ 5 seconds
